Usually, you'll need to change the extension of your page to .php (instead of .htm) for the server to parse the page as PHP.

How can I make a navigation toolbar, between Normal, TOC, blog Mode?

You can copy the source from the demo page on this site.

Basically, there are three pages, each integrated with a particular calendar type. Then, the navigation bar is just a link to each of the three pages.
